Register As A New Patient?
All new patients are required to undergo a health check when they register with the surgery.
New patients without an NHS card are required to bring:
- proof of address
- proof of ID (eg driving licence, passport). Also, bring any medication if you are currently taking any.
(New patients with an NHS card are required to bring proof of address only.)
Please note: It is practice policy not to discriminate against any person on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Thank you for your application to register with Old Oak Surgery. It would be useful for us to know a little more about your medical history in order to help you. Could you therefore please download and print out the
questionnaire and answer the questions as accurately as possible?
